Chiedu Nchekwube, MD, FAAFP, ABIHM
Dr. Nchekwube is the founder of the Center. He is board certified and specialized in Family Medicine and also board certified in the sub-specialty of Integrative Medicine. He has been practicing Integrative Medicine since 1988. Dr. Nchekwube has been a guest on many radio and television programs. He is a clinical assistant professor of family Medicine at the University of Illinois in Chicago and Indiana University Medical Schools. Dr. Nchekwube researches and formulates disease-specific nutraceutical products. He modified and teaches meditation techniques for self- healing.
Dr. Chiedu Nchekwube practices what he preaches. He has been practicing yoga since the age of 17 and has been an avid meditator for over 35 years. He shares his experiences with the public and his patients in a very comedic and casual manner thereby facilitating learning in relaxed environment.
Ngozi Nchekwube, JD
She is the COO of Family Medicine and Wellness Center and has managed the center since its inception in 1997. The Center operations depends on her able guidance and business prudence. The impetus for many of the programs in the center derived from her practicing what the center preaches. With her deep understanding of integrative medicine, she is able to mirror the patients’ experiences making it easy to smoothly facilitate the Implementation of most of the programs in our center.
Juanita Golden-Allen, AGACNP
I am pleased to announce the addition of Juanita Golden-Allen, AGACNP to our growing practice. Juanita Golden-Allen is an Adult Gerontology Acute Care Nurse Practitioner, with over a decade of experience providing medical care to those in need. She has a unique focus on wellness and preventative care, and likes to work in partnership with her patients.
She originally started working in the medical field as an Emergency Medical Technician for the Gary Fire Department. She had a desire to grow in the medical field, so she became a Licensed Practical Nurse; giving her the opportunity to work in nursing homes addressing major needs in the geriatric population. She received her degree in Science of Nursing from University of Saint Francis. As a Registered Nurse she worked at Methodist Hospitals. She enjoyed providing critical care to ICU patients and comforting many families. After getting her Master’s degree from Walden University, Juanita became an Adult Gerontology Acute Care Nurse Practitioner.
She is passionate about decreasing the health disparities that exist in health care. She is culturally diverse, and feels everyone should be respected and provided with adequate health care. Juanita also values proper patient education on topics like; diabetes, obesity, asthma, nutrition, weight loss, hormonal imbalances, and overall wellness to heal the body and mind.
Nurse Practitioners diagnose and treat illnesses; and are able to order and interpret tests, write prescriptions, and do many common in-office procedures. She has a unique focus on Wellness and Preventative Care; and likes to work in partnership with her patients, just as I do.
Personal Information: Juanita grew up on the South Side of Chicago, IL.. She later moved to Northwest Indiana to raise her own family. She is married, has three sons, and two small dogs Ci-Ci and Jada. Her favorite color is blue; her favorite food is Mexican; and her favorite hobby is listening to music. She loves getting to know people and makes it easy for those around her to share their personal feeling with her.

Leanna wears many hats in the practice. She makes things happen from the inception of any idea to its implementation in the practice. If anyone needs anything in the practice she is the go to person.

Erica supervises the front & back clinical offices. When it comes to getting in and out of the office for services, Erica is the power behind the flow in the office from the front office to the clinical section. When the patients need help, it is Erica and her team that are available to respond either in person or through electronic media.
Marisa is our coder and accuracy officer. She makes sure that the codes are correct for the record and the insurance companies.
